Services
Medical Services:
Our team provides Primary Care medical services, which include general medical care, behavioral health services, and addiction treatment. These services are provided by licensed medical providers, Medical Doctors, Physician Assistants, Nurse Practitioners, and Nurses. We serve patients experiencing homelessness through an innovative “street medicine” approach aimed at meeting a person where they are and caring for a person’s comprehensive needs. Our medical team works together with our care management and community support staff to provide for these comprehensive needs.Enhanced Care Management:
Enhanced Care Management (ECM) is a Medi-Cal benefit available to those experiencing homelessness that will address clinical and non-clinical needs of the highest-need patients. Each patient works with a Peer Navigator to coordinate care and services to address their complex social and health needs. This involves arranging and facilitating follow up and specialty appointments, arranging transportation, and working closely with clients one-on-one. This is designed to provide a whole-person approach to care and is a collaborative and interdisciplinary approach to providing intensive and comprehensive care management services.
Community Supports:
We work to connect individuals with Community Supports programs to meet their social needs, specifically housing navigation and housing deposits. Community Supports are services provided through Medi-Cal managed care plans and are designed to address social drivers of health. Our Peer Housing Navigators work directly with our clients to help address housing barriers and barriers to remaining housed in collaboration with our integrated HIA team and other key local community partners.
